The offered sheet is signed by members of the band Blind Melon, including lead singer Shannon Hoon, who passed away at only 28 years old on October 21, 1995, of a drug overdose. Band guitarist Brad Smith and drummer Glen Graham have also signed the sheet. Hoon has also likely written the band name, "Blind Melon," as it is aligned with his signature. The band's 1992 debut album, Blind Melon, went multi-platinum on the strength of the single "No Rain." The band's second album, 1995's Soup, was released just three months prior to Hoon's untimely passing.

The memo pad sheet is from "The Pfister Hotel," which is located in downtown Milwaukee and whose logo on the sheet indicates that it celebrated its 100th anniversary in 1993. The band could have stayed there for either of their two tour stops in Milwaukee in 1993: February 26 at Marquette University's Alumni Memorial Union or on August 14 at the Marcus Amphitheater where they open for Neil Young. Accompanied by a letter of authenticity from Beckett Authentic. The sheet measures 4 1/4 x 6 1/2 inches and presents with no wear and just the slightest waviness. Near Mint condition.
