The offered "FORM W-4" is signed by Hall of Famer Sandy Koufax in bold blue felt tip. The form is marked as "Rev. Jan. 1967," making this form for the first year that Koufax was retired from Major League Baseball and the Los Angeles Dodgers. His stellar yet injury-shortened career would make him the youngest player since Lou Gehrig to be put in the Hall of Fame in 1972. After having six consecutive dominant seasons where he won 3 Cy Young awards, Koufax was forced to retire citing "arthritis" in his pitching arm as the reason. It may have something to do with the fact that he threw more than 650 innings and 54 complete games in his last two seasons alone--by comparison, most starters today would take 3 to 5 years to amass that many innings! Also included is a modern 5 x 7 inch print of the cover of the August 2, 1963 issue of Life Magazine. Accompanied by a letter of authenticity from Beckett Authentic. The form measures 8 x 3 1/2 inches and presents with only minor wear, and Koufax's SS# has been taped over. Near Mint condition.
