Offered is the San Francisco 49ers “1982 Public Relations Questionnaire” filled out by NFL Hall of Famer Joe Montana. Montana's handwritten answers include his hometown “Monongahela, PA”, his wife “Cathleen Castillo” and the city and state of hisl college, “South Bend, Ind”. South Bend, of course, is the home of Notre Dame University where Joe won a National Championship prior to his 16-year NFL career. He won four Super Bowls, three Super Bowl MVPs, and still hold records for the game that include most passes without an interception (122 in four games) and the all-time highest passer rating of 127.8. The form is accompanied by a letter of authenticity from Beckett Authentic. The sheet presents with some minor stains and toning. Excellent to Mint condition.
